According to the American Cancer Society Facts and Figures (2010), African American women have an increased incidence of breast cancer diagnosis before the age of 45, in comparison to Caucasian women who have an increased incidence of breast cancer diagnosis after the age of 45. Breast cancer incidence rates for 2002-2006, showed that 97% of breast cancer mortalities were of women under the age of 40. These rates of incidence of breast cancer became striking when put together with data showing that death rates per 100 000 in Caucasian women is 21.7- 27.3; while their African American counterparts show a death rate of 20.9-40.0 per 100 000 and more than 190 000 new cases of invasive cancer in 2009 (American Cancer Society, 2010). It is the role of the Doctorate of Nursing Practice (DNP) in addition to the Healthy People 2020 initiatives to provide breast cancer education and awareness campaign to decrease the percentage of women who lack the adherence to the ACS breast cancer screening guidelines by utilizing the Breast Risk Inventory Evaluation (BRIE) survey.

The research will integrate the PICO method with Dorothea Orem’s self-care theory. The research will then try to prove that with proper education and intervention, more people will be aware of the breast cancer screening procedures. With the knowledge of the screening and the adherence to the procedures, early diagnosis and prevention of breast cancer can be achieved and the mortality rate of the patients will be lessened.

Purpose and Statement of Problem

This study is designed to answer the research question: can culturally-sensitive education decrease the incidence rate of breast cancer diagnosis in African American women? Parallel question will be: with early detection and screening will we be able to stop the progression of the disease and eliminate risk factors leading to mortality due to breast cancer? The Alma Ruth Project is named for evangelist Alma Ruth Wilson, formerly of Forest Manor United Methodist Church in Indianapolis, Indiana. She fought a long hard battle with breast cancer; however the battle ended on August 23, 2007. The goal with the Alma Ruth Project is to provide African American women with breast cancer education and screening information. Dorothea Orem’s self-care theory will be integrated with the Patient Intervention Comparison Outcome (PICO) methodology for critical appraisal of the research design.
